Gellert Health navigators are highly trained individuals in helping one navigate their healthcare journey. We walk hand in hand with each of our patients to ensure the care team is hearing the patients’ values and needs, and that all needs are followed up on. Our goal is to ensure that every Gellert Health patient feels safe, healthy, and confident in expressing their goals to achieve one’s best self.

The Social Determinants of Health
The Social Determinants of Health (SDOH) play a significant role in a persons health outcomes. The SDOH are the economic and social conditions that influence individuals and group differences in health status. Gellert Health either directly addresses or ensures that each patient is connected to resources supporting the social determinants of health.
How it Works
Gellert Health has redesigned healthcare for vulnerable populations by addressing the population’s greatest barriers: lack of understanding of how to navigate the healthcare system and lack of transportation.
