Integrity + Quality + Kindness.
Navigating the healthcare system alone can be difficult and often leave us feeling overwhelmed and confused. Gellert Health navigators are highly trained individuals in helping one navigate their healthcare journey. We walk hand in hand with each of our patients to ensure the care team is hearing the patients’ values and needs, and that all needs are followed up on. Our goal is to ensure that every Gellert Health patient feels safe, healthy, and confident in expressing their goals to achieve one’s best self.
Gellert Health always meets our patients where they are. We assist our patients with:
- Achieving physical and emotional wellness
- Being creative around remembering to take medications and attending appointments
- Attending all scheduled appointments with the patient
- Offering transportation for all patients to medical and behavioral health appointments
- Providing opportunities for social interaction
- Educating our patients around health literacy and timely/appropriate utilization of resources in healthcare
The Social Determinants of Health
The Social Determinants of Health (SDOH) play a significant role in a persons health outcomes. The SDOH are the economic and social conditions that influence individuals and group differences in health status. Gellert Health either directly addresses or ensures that each patient is connected to resources supporting the social determinants of health.
How it Works
Gellert Health has redesigned healthcare for vulnerable populations by addressing the population’s greatest barriers: lack of understanding of how to navigate the healthcare system and lack of transportation.
- Connecting patients to appointments
We connect & support patients with medical and behavioral health appointments, tele-health appointments, physical therapy, surgeries and procedures, and ongoing treatments for chronic conditions.
- Considering social determinants of health
We meet individuals where they are at while considering social and economic challenges that influence differences in health status.
- Improving health outcomes
We influence social cohesion and improve health outcomes. Through a caring and efficient health navigation and transportation system, the communities most vulnerable are navigated back to health.
