Our Navigators support mothers through a safer, smoother pregnancy by promoting their physical and mental well-being and enhancing health outcomes from prenatal to postpartum care. We also connect patients with helpful resources. Our services are covered by eligible health insurance plans.
How It Works
Support With Appointments
We will get to know your healthcare needs, help you schedule and attend appointments. We’ll also take notes to help you remember what was said.
Prenatal Care
We will support you through prenatal care, making sure you get to appointments, labs, and screenings on time — and help you find resources you need.
Postpartum Care
Care doesn’t stop at birth and neither do we. We will continue walking with you through recovery so both you and baby have a strong, healthy start.
Coordinate Transportation
Need help getting there? We can help you set up transportation to and from appointments so you arrive safely and on time.
No Cost
This service is 100% covered through eligible insurances. You will not be responsible for any cost of services provided by Gellert Health.
Eligibility
- Enrolled in eligible insurance.
- You are 18 or older
- You live within 30 miles of Downtown Phoenix
Need Support?
We’re Here for You
This program is by referral-only, which means a referral from your healthcare provider is required. This may be your primary care doctor (PCP), OBGYN, a case manager or other authorized healthcare provider. Ask your healthcare provider for a referral if you think our services are a good fit for you!

Eligible Health Insurance
Gellert Health accepts the following AHCCCS (Medicaid) plans:
- Mercy Care
- Mercy Care ALTCS
- Mercy Care RHBA (Title 19 only)
- Molina Health Care
- United Healthcare Community Plan
- United Healthcare ALTCS
Medicare Plans are accepted ONLY WHEN there is secondary coverage by one of the AHCCCS Plans listed above.
